The Recovery Support Navigator is responsible for Case Management, Network Support Facilitation, and Case Coordination within our community-based initiative.

Salary: $17.50/hour with productivity incentives.

Duties & Responsibilities

  • Meet with client face to face
  • Develop a set of goals and objectives identified by client
  • Implement goals and objectives in comprehensive care plan/family support plan
  • Deliver services to client in the community (home, treatment facility, diversionary unit)
  • Obtain insurance authorizations and conducts concurrent reviews
  • Assist client with transportation needs
  • Arrange referrals for services when needed
  • Coordinate care with inpatient staff, aftercare facilities and community partners
  • Provide linguistically appropriate and culturally sensitive recovery support navigation
  • Document all services in electronic health record
  • Participate in program meetings and supervision sessions
  • Share 24/7 on call
  • Utilize Arise model with clients and their families when applicable 

Experience and Skills:

Education & Experience

  • Bachelor’s Degree in a related field preferred, but will consider lived experience
  • Must have own transportation, valid license, and proof of insurance
  • Strong evidence of effectiveness in engaging clients
  • Solid Knowledge of SSTAR services as well as community resources
