is a Federally Qualified Health Center (FQHC) which affords our
employees eligibility to apply for one of our three Loan Repayment
Programs.
The Health Access Navigator is responsible for assisting individuals
to identify, enroll, and access affordable health insurance options
through Medicaid and the Health Insurance Marketplace.

Duties & Responsibilities
- Knowledge of eligibility requirements for Medicaid, Medicare,
other alternate medical and social service programs/benefits available
- Understands the process for eligibility and submission of health
insurance applications
- Ability to monitor and complete reports to funders to support
enrollment and outreach programs.
- Knowledge of NextGen EMR system to document income verifications
and update demographics of health center patients.
- Monitor enrollment status of clients and families to ensure
completion of application process.
- Participate in outreach and advocacy activities that promote
available services
- Assist qualified clients for enrollment into the Health Insurance
Marketplace application program ensuring applications are completed
and submitted accurately
- Ensures compliance with HIPAA and the Privacy Act when collecting
or releasing patient information including patient personal,
financial, and medical confidentiality
- Attend classes, seminars, and workshops in order to maintain
current knowledge of public benefit programs and web-based benefits
application sites.
- Demonstrates excellent patient relations skills on the phone and in-person,
- Provide education to patients and staff on the Sliding Fee Scale
Program requirements and eligibility
- Maintain accuracy of related policies, procedures, forms, and
materials that support of the Sliding Fee Scale Program requirements
(e.g., registration and scheduling, financial eligibility,
screening, enrollment, patient notifications, billing, and collections.
- Cross-train and build awareness of community health worker
services among the health care team and provide assistance when needed.
