SSTAR’s Fall River Inpatient Program has an opening for a Part Time Case Manager, 24 hrs per week, M, W and Friday.. We are looking for a motivated individual to provide   case management services to connect clients to after care treatment. It is essential that this person contributes as a team member to produce an environment conducive to the successful treatment of patients. Bi-lingual people of all genders are encouraged to apply.

1. One on one case management of CSS clients to facilitate transition back to the community and next level of care.

2. Awareness of community resources and referral mechanisms.

3. Identify gaps in service and advocate appropriately.

4. Develop relationships with halfway houses, shelters, sober housing and transitional housing.

5. Arrange transportation to appointments and interviews.

6. Engage formerly estranged families/concerned others with aftercare planning.

• Bachelor’s Degree in social work or related field preferred or significant experience and training in the substance abuse and mental health field.

• Strong verbal and written communication skills.

• Open to working with people from diverse backgrounds, cultures and lifestyles in a non-judgmental manner.

• Direct care positions in addiction programs require two years of continuous sobriety for individuals in recovery.
