Job Description:

Join SSTAR’s Fall River Inpatient Team as a Case Manager (32 hrs/week)

Are you passionate about helping individuals build a path to lasting recovery? SSTAR’s Fall River Inpatient Program is seeking a dedicated and motivated Case Manager to join our team!

In this rewarding role, you’ll play a vital part in supporting patients as they transition from inpatient care to continued treatment and community resources. You’ll work closely with a collaborative, compassionate team to ensure each individual has the tools and connections they need for success beyond our program.

What You’ll Do

  • Provide comprehensive case management services focused on aftercare planning
  • Connect clients with ongoing treatment, housing, and community resources
  • Collaborate with a multidisciplinary team to support patient progress
  • Help foster a safe, supportive, and recovery-focused environment

Experience and Skills:

  • Education: High school diploma or GED required; Bachelor’s degree in Social Work or a related field preferred.
  • Strong verbal and written communication skills.

  • Open to working with people from diverse backgrounds, cultures and lifestyles in a non-judgmental manner.

  • Direct care positions in addiction programs require two years of continuous sobriety for individuals in recovery.
