Job Description:

Provide intensive case management and care coordination services to high-risk individuals/priority populations

Specific Responsibilities

  1. Screen clients to determine eligibility for SOR Grant
  2. Meet with clients and create a wellness plan
  3. Develop a set of goals and objectives identified by client
  4. Implement goals and objectives in integrated service plan
  5. Deliver services to client in the community (home, treatment facility, diversionary unit)
  6. Obtain insurance authorizations and conduct concurrent reviews
  7. Assist client with transportation needs
  8. Arrange referrals for services when needed such as primary and behavioral healthcare
  9. Coordinate care with inpatient staff, aftercare facilities and community partners
  10. Provide linguistically appropriate and culturally sensitive case management services
  11. Document all services in EHR
  12. Complete all required documentation for DPH, complete all required documentation in REDCAP e.g. GPRA baseline, follow-ups, service logs, discharges, etc.
  13. Participate in MAT and SOR program meetings and RSN supervision sessions
  14. Participate in trainings as it relates to opioid use disorder, stimulant use disorder, MAT, etc.
  15. Provide meaningful links to treatment, recovery support services, and mutual aid
  16. Form therapeutic relations with individuals affected by SUD
  17. Provide intensive care management upon admission and keep in close contact until the individual reaches stability
  18. Provide patient education on community resources and programs
  19. Provide target interventions to priority populations such as young adults, women with children, and recently incarcerated individuals
  20. Other duties as required

Experience and Skills:

Qualifications

  1. High school diploma required, Bachelors degree desirable
  2. Strong organizational skills
  3. Solid written and oral communication skills
  4. Ability to effectively engage clients
  5. Knowledge of SSTAR services as well as community resources
  6. Available to respond by phone or in-person after business hours
  7. Must have own transportation
  8. Previous experience in the field of health care, medical care, or public health preferred

*At least 2 years continuous sobriety is required for people in recovery from drugs and alcohol
