Job Description:

SSTAR’s Fall River Inpatient Program has an opening for a full time (40hrs) case manager. The hours are Monday - Friday 8:00am - 4:00pm. We are looking for a professional who can advocate for clients to coordinate aftercare planning. Strong communication and documentation skills are required as well as the ability to form positive work relationships with other substance use disorder agencies across the state.

Specific Responsibilities:

 

  1. One on one case management of CSS clients to facilitate transition back to the community and next level of care.
  2. Awareness of community resources and referral mechanisms.
  3. Identify gaps in services and advocate appropriately.
  4. Develop relationships with long term residential treatment programs, shelters, sober housing and transitional support services.
  5. Arrange transportation to appointments and interviews.
  6. Engage formerly estranged families/concerned others with aftercare planning.
  7. Maintain client confidentiality by complying with 42CFR part 2.
  8. Represent programs internally and externally as requested.
  9. Accept and participate in appropriate supervision.
  10. Other duties as requires.

Experience and Skills:

Qualifications:

  • High school Diploma
  • Strong verbal and written communication skills.
  • Open to working with people from diverse backgrounds, cultures and lifestyles in a non-judgmental manner.
  • Direct care position in addiction programs require two years of continuous sobriety for individuals in recovery.
