Job Description:

SSTAR is hiring a full time Case Manager for the Intensive Outpatient Program.  Currently all IOP groups ar facilitated through Zoom.  Employees are on campus, this is not a remote position.  First Shift, $16.50 per hour.

At SSTAR, we recognize the chronic nature of chemical addiction and support clients in their pursuit and maintenance of sobriety. IOP staff provide a structured multidisciplinary program of addiction treatment and supported referrals to other services. We strive to provide skills-based education that is relevant and useful to the client so that they can move forward.  

Specific Responsibilities:

  • Obtain initial insurance authorization
  • Obtains extensions for insurance authorization
  • Check daily attendance
  • Facilitate groups if needed
  • Complete EIM/ESM’s upon admission
  • Document all collateral contacts and services in NexGen
  • Track and collect fees
  • Meet with clients to develop and review treatment plans
  • Link clients to appropriate services, such as primary care, transportation, psychiatric
  • When discharge is planned, arrange for aftercare appointments
  • Phone or letter outreach for clients who drop out
  • Participate in daily team meetings
  • Markets the program
  • Participate in ongoing development of curriculum for IOP
  • Provide one-on-one counseling
  • Complete discharge summaries/Disenrollment MIS
  • Answer phone inquiries about the program from prospective client and referral sources
 

 

Experience and Skills:

Qualifications:

  • BA in related field or CAC/CADC preferred
  • Demonstrated effectiveness in working with and engaging an addicted population
  • A minimum of two years of experience in substance dependence and/or mental health field
  • Strong organizational skills
  • Ability to function independently with minimal supervision
  • Solid understanding of insurance procedures and payor system
  • Knowledge of community services - especially SSTAR services
  • Direct care positions in addiction programs require two years of continuous sobriety for individuals in recovery
