The Case Manager is responsible for clinical and medical case management for all Lifeline clients.

Duties & Responsibilities

  • One on one case management for person with substance abuse disorders.
  • Active participation as a member of the multi-disciplinary team that develops and supports the client’s service plan.
  • Coordinate care/services with SSTAR open access model and primary care needs.
  • Awareness of community resources and referral mechanisms.
  • Identifies gaps in service and advocates appropriately.
  • Maintain confidentiality to the fullest extent.
  • Assist with transportation arrangements.
  • Represent program/agency internally and externally as requested.
  • Accepts and participates in appropriate supervision.
  • Other duties as required.

Experience and Skills:

Education & Experience

  • Bachelor’s degree in social work or related field, preferred.
  • CADC, preferred.
  • Strong verbal and written communication skills.
  • Open to working with people from diverse backgrounds, cultures, and lifestyles in a non-judgmental manner.
