Job Description:

SSTAR's Community Health Worker will assist in providing non-clinical case management services to clients in the MAT Program. The CHW also helps educate the community in a continuous effort to destigmatize Medication Assisted Treatment.

  • Assist clients by providing referrals to housing, transportation, benefits programs, and other programs and agencies that can assist the client.
  • Triage/ Screen for possible referral into the MAT program.
  • Provide community outreach and education to ensure that local providers, agencies, and community members are aware of the integrated primary care provided in the Medication Assisted Treatment Program.
  • Assist clients in obtaining care in the SSTAR Family Health Center.
  • Setting up trainings for other agencies with staff from the MAT presenting information to help agency staff better understand Medication Assisted Treatment and make appropriate referrals.
  • Assist providers with MassPat Compliance.
  • Assist in planning and implementation of "Medication Assisted Treatment Day" each September as part of the Recovery Month activities held during the month.
  • Prior Authorizations for Buprenorphine products as well as Injectable Naltrexone.
  • Other duties as required

Experience and Skills:

  • High School Graduate with two years of full-time, or equivalent part-time, professional or para- professional experience in the field of health care, medical care, or public health education in a recognized hospital, medical facility or agency.
  • Organizational skills.
  • Communication skills.
  • Bi-lingual in Portuguese or Spanish preferred.
  • Vehicle with a valid Massachusetts motor vehicle operator’s license.
  • At least two years continuous sobriety is required for people in recovery from drugs or alcohol
